Create a mechanism for a non-registered user to submit a document

Instead of making people register with github and create a github issue to suggest a document at a URL, create a web form similar to the URL Wizard (or maybe enhance URL Wizard) to allow non-registered users to submit a document with associated metadata. This goes into a table where a registered user can approve the submission with possible corrections.
