"Index Set Strategy" disables submit button inconsistently

Expected Behavior

The submit button in the "Index Set Strategy" should be disabled consistently.

Minor Details: The dialog title should be "Index Set Template", matching the content the user is configuring here.

Current Behavior

In the dialog where an index set template is selected, the "Apply template" button is disabled by default. Through switching tabs und selecting a template in the "Custom Templates" tab, I can enable it, even when the warm tier checkbox is checked.

Steps to Reproduce (for bugs)

1. Run Open Core without Enterprise Plugin
2. Go to System/Indices
3. Click "Create Index Set"
