Sorry if I write here to much, I just want to help to make this great plugin to better.
I noticed something with the "One per category".
When I set the Data format: Category and I leave unchecked the "One per category" as default, that is behaves like is be checked. I set 3 category to show in 5 rows but first is shows only 3 posts and each from one a category.
So, I checked the "One per category"-> save, after that unchecked and save. And now is shows in all 5 rows.
Actually, the Link Images is behaves like same, too.
Can you guys, check this?

Are you able to reproduce it on the all new instances of the widget? I've tried to do it - I've selected one category, then set the widget to display 3 cols and 2 rows, and I've got 6 items from the specified category without touching the "One per category" option which is defaultly unchecked.

No, actually I only a few cases this experience. But I don't know the reason. If you can't reproduce this, than just the problem is with my theme.
But in the link images case, I get always this.
Widget layout: Article wrapper: Default
Pages: 0 Columns: 0 Rows:0
Links block:
Pages:1 Rows: 10
Link format: only the "Show Title" is checked, the "Show Image" is unchecked
