Allow multiple questions and to also have an option whether to use multiple choice or multiple answers

This is an awesome extension. It would be great for this to allow asking multiple questions instead of just one. For example, I would like to open a discussion about a certain social issue and I have several related questions that I want people to answer (something like a survey). This extension is going to be useful if it has that feature.

This one currently does multiple choice. Consider also expanding it to allow multiple answer type of questions. I think this is similar to Issue #3 on enhancement.
