A lot of things in Hamlet interiors used a special type of random-direction item drop that only works in roughly a cone towards the screen, which seems to have been created to prevent items from being thrown backwards behind objects and lost.

There seems to be code for this in the End's Well. It appears to only drop Purple Gems in that front-facing cone.
