The first post in a gallery thread is considered to be the "cover" of the gallery and is not displayed by default. This is not always intuitive, and generally people miss the "show cover" option.

A potential solution is to make marking the first post as a cover in gallery mode an explicit action, or something that can be triggered on and off by the thread creator.
