[Enhancement] Impose hierarchical outline structure on TOC in PDF export #3

Problem
The table of contents in the PDF export of docs.bl.org is flat, and does not include outline headings.

Desired behavior
The table of contents in the PDF export should mirror the outline structure in the left-hand navigation bar of docs.bl.org.
