[slack-app] Multi-app support

Description

Currently, each Slack app is hosted by a dedicated Azure Container app. While there are some benefits to this approach, it should also be possible for a single backend instance (Container app) to host multiple Slack apps.

To support this possibility, a few configuration settings should be moved from environment variables to either db-config or url parameters.
