For key feature 3 listed in the User Guide:

Perhaps, it can be made clearer how patient records can exist from multiple departments, since multiple departments likely have multiple doctors, and each doctor belongs to 1 department.

Here are some suggestions:
If the app is meant for a single doctor: We may keep track of the department patients belong to.
If the app is meant for multiple doctors to use at once: An appointment can be additionally tied to a doctor.
